If you’re uninsured or underinsured in the Lowcountry, you still have options for medical care. They’re not always easy to find, and they’re not perfect substitutes for real insurance. But they exist, they serve real people every day, and knowing about them can be the difference between getting care and going without. This is a practical directory of resources available in the Summerville, North Charleston, Goose Creek, and greater Charleston area.

Community Health Centers

Federally qualified health centers (FQHCs) are required to see patients regardless of ability to pay. Fees are based on a sliding scale tied to your income and family size. Even at full price, they’re dramatically cheaper than private practices or hospital clinics.

Fetter Health Care Network

The primary FQHC serving the Lowcountry. Multiple locations across the region.

Services: Primary care, pediatrics, OB/GYN, dental, behavioral health, pharmacy, substance abuse treatment, HIV/AIDS services, lab work

Sliding scale: Based on Federal Poverty Level guidelines. Patients at or below 100% FPL may pay as little as $20 per visit. Patients above 200% FPL pay a discounted fee.

Little River Medical Center

Serves the Waccamaw region and parts of the Lowcountry with full primary care and dental services on a sliding scale.

Free Clinics

Free clinics provide care at no cost to qualifying patients. Eligibility typically requires being uninsured and meeting income guidelines.

Volunteers in Medicine (VIM)

VIM is staffed by retired physicians and volunteer healthcare workers. Wait times for new patient appointments can be several weeks, but established patients are seen regularly.

Barrier Islands Free Medical Clinic

Location: 3226 Maybank Hwy, Johns Island, SC 29455 Eligibility: Uninsured residents of Johns Island, Wadmalaw Island, and James Island below 200% FPL Services: Primary care, chronic disease management, medications, lab work, referrals Contact: 843-266-9800

Hospital Financial Assistance Programs

Every nonprofit hospital is required by federal law to maintain a financial assistance (charity care) policy. These programs can reduce or eliminate hospital bills for qualifying patients.

MUSC Health Financial Assistance

Coverage: Patients at or below 200% FPL may qualify for free care. Patients between 200-400% FPL may receive discounted care. Applies to: Inpatient, outpatient, emergency, and physician services at MUSC facilities How to apply: Contact MUSC Financial Counseling at 843-792-3682. Apply before or after receiving services. They can work with you on payment plans even if you don’t qualify for full charity care. Facilities: MUSC Health University Medical Center (Charleston), MUSC Health locations throughout the region

Roper St. Francis Healthcare

Charity Care Program: Income-based financial assistance for uninsured and underinsured patients Eligibility: Generally available to patients below 300% FPL How to apply: Contact the Patient Financial Services department at the facility where you received care. Bring proof of income and residency. Facilities: Roper Hospital (Charleston), Bon Secours St. Francis Hospital (West Ashley), Roper St. Francis Mount Pleasant Hospital, and affiliated physician practices

Trident Health System (HCA)

Financial Assistance: Available through HCA’s charity care program Eligibility: Income-based; contact the billing department for specific thresholds Facilities: Trident Medical Center (North Charleston), Summerville Medical Center (Summerville) How to apply: Call the facility’s billing department or visit the financial counselor’s office. Applications are available online.

Do not ignore hospital bills. Even if you can’t pay, apply for financial assistance before the bill goes to collections. Hospitals would rather work with you than send your account to a collection agency.

Prescription Assistance

Medication costs can be the biggest barrier to managing chronic conditions. These resources can dramatically reduce what you pay.

Pharmaceutical Manufacturer Programs

Most major drug companies (Pfizer, Lilly, Novo Nordisk, AstraZeneca, Merck, etc.) offer patient assistance programs (PAPs) that provide free or deeply discounted medications to qualifying patients. Each program has its own income requirements, typically 200-400% FPL.

How to apply: Through NeedyMeds.org or RxAssist.org, which maintain searchable databases of every PAP. Your doctor may need to submit paperwork on your behalf.

GoodRx

Free prescription discount card accepted at virtually every pharmacy in Summerville. Discounts range from 10% to 80% off retail prices. No income requirements, no registration. Just pull up the coupon on your phone at the pharmacy counter.

Mark Cuban Cost Plus Drugs

Online pharmacy offering medications at cost plus a flat markup. No insurance needed. Many common generics are $3-$10 for a 30-day supply. Check costplusdrugs.com for your specific medications.

Walmart and Publix $4 Generics

Both chains offer select generic medications for $4/30-day supply or $10/90-day supply. No insurance required. Ask the pharmacist for their generic list.

SC DHEC Prescription Assistance

South Carolina’s Department of Health and Environmental Control offers some prescription assistance through local health department offices for specific conditions, including HIV/AIDS, STIs, and tuberculosis.

Dental Care

Adult dental care is one of the hardest things to find affordably. Most insurance plans don’t cover it adequately, and dental work is expensive.

MUSC College of Dental Medicine

Provides free dental care to elderly, disabled, and medically fragile individuals who can’t afford treatment. Application process takes time, but the care is provided by volunteer dentists at no cost.

Support groups and educational programs for individuals and families affected by mental illness. Free to attend.

Palmetto Lowcountry Behavioral Health

Inpatient and outpatient behavioral health services. Financial assistance available.

Women’s Health

SC DHEC Family Planning

Low-cost or free reproductive health services including contraception, STI testing, and Pap smears through local health department clinics. Available in Dorchester, Berkeley, and Charleston county health departments.

Planned Parenthood

Reproductive health services on a sliding-scale fee basis. Services include contraception, STI testing and treatment, cancer screenings, and general gynecological care.

Children’s Health

SC Healthy Connections (Medicaid) and CHIP

Children in families earning up to 208% FPL are eligible for free or low-cost coverage through Medicaid or the Children’s Health Insurance Program. Apply through SC DHEC or scdhhs.gov. There is no coverage gap for children. If a family qualifies, the children are covered.

Help Me Grow SC

Connects families with children birth to age 8 with developmental services, family support, and health resources. Call 211 or visit helpmegrowsc.org.

  1. Start with Fetter Health Care Network for primary care. They’re the front door to the Lowcountry’s safety net.
  2. Apply for hospital financial assistance before you get a bill, if possible. If you already have a bill, apply immediately.
  3. Use GoodRx or Cost Plus Drugs for prescriptions. Check prices before filling at a retail pharmacy.
  4. Call 211 for referrals to any social service. They maintain a full database of local resources.
  5. Don’t skip care because you think you can’t afford it. These programs exist because the system recognizes that not everyone can pay full price. Use them.

Go to the nearest emergency room. Trident Medical Center in North Charleston and Summerville Medical Center in Summerville are the closest to the Summerville area. EMTALA requires treatment regardless of ability to pay. Apply for financial assistance afterward.
